EPC honors our clients’ right to their health information. If you’d like a copy of your protected health records, please submit all requests using the following information:

 Requesting Records under the Public Information Act, Texas Government Code, Chapter 552 

The Department of State Health Services is committed to providing full access to public information. To request records under the Texas Public Information Act: 

  • Submit a request in writing via U.S. Mail, fax or email. 
  • Include contact information and a clear description of the records you are requesting.

You will be notified in writing if there are costs associated with filling your records request. The Office of the Attorney General sets what a state agency may charge for information.
